Only those medications which are necessary to maintain the student in school and which must be given during school hours should be administered. Any student who is required to take medication during the regular school day or while participating in school sponsored activities (i.e. field trips, athletics) should comply with the below procedure.

1.  Medications may be administered in school only in compliance with New York State Education Department regulations.
2.  Medications will only be given with a written order from a license health care provider and with written permission from the parent / guardian of the student.
3.  The physician’s order can be on a prescription or they can complete the bottom half of the Medication Administration Form. The information must include:

- Diagnosis or condition being treated
- Dosage, frequency, and route of administration
- Information regarding the drug, such as it’s usage and possible side effects
- Physician’s name, address, telephone number and license number.

1.  The medication must be in its original container bearing the pharmacy label. Pills placed in an envelope or separate container can not be administered by the nurse.
2. The medication will be kept in the Health Office in a locked cabinet.
3.  It is strongly recommended that all medication that can be given outside of the school hours, without deleterious effects to the child, be given at home.
4.  Only medication delivered to the School Nurse by the parent / guardian will be accepted.
5.  All medication must be picked up at the Nurse’s Office prior to the last day of school or it will be discarded.
